After you have had a chance to review the information below, please let me know if you would like to move forward with services by simply completing and returning the Policies and Agreement (p.2), Consent to Release Information (i.e., authorization for support providers that you think would be helpful for me to connect with, such as school) and Parent Intake forms and then send/ scan them back to me electronically, along with any other information that you feel would be helpful for me in getting to know your child (i.e., any academic or cognitive testing, neuro/psychoed. evaluations, related service reports, etc.).
